The Swing Bed Program is a short-term rehabilitation program for patients who no longer need acute care hospital services, but need to increase their strength and independence prior to returning home from a hospital stay.
Patients receive daily therapies, if indicated, to work toward meeting self-care goals set by the patient and his/her healthcare team. The team also assists with all aspects of discharge to ensure a safe and comfortable transition from hospital to home.
Unlike acute care, where treatment is ordered by a physician based on your medical diagnosis; swing bed care is rehabilitation provided by an interdisciplinary team to help you reach specific goals for self-care. Patients wear their own clothes and eat meals in the activities room.

To be eligible for a swing bed:
- You require a “skilled” level of care
- A qualifying hospital stay
- You agree to participate in short-term rehabilitation
During your extended stay, you may see some/all of the following healthcare professionals:
- self-selected physician(s)
- hospitalists
- registered nurses
- swing bed coordinator
- social worker/discharge planner
- pharmacists
- dietitians
- physical, occupational and speech therapists
- activities coordinator
- Medicare specialists
- behavioral health specialists
