Patient Forms and Notices

At MaineHealth, the privacy of your health information is a top priority. All MaineHealth locations follow strict guidelines that secure your medical records in accordance with your rights, and with federal and state regulations. You, or your legal representative, have the right to inspect and obtain a copy of your medical records. We do not offer walk-in services but can provide records electronically, on paper, on a USB flash drive, or on a compact disc (CD). Upon request, we will also send a copy of your medical records to another health care provider.

Youth Peer Support Statewide Network - The Youth Peer Support Statewide Network (YPSSN) is a new, growing resource open to any young adult in Maine.

We recommend the following items to help make a behavioral health inpatient stay more comfortable:

  • Slip-on style shoes
  • Pajamas (without drawstrings)
  • Several changes of clothes (without drawstrings)
  • Books of non-violent/non-sexual nature
  • Cordless radio, journals, stuffed animal
  • Glasses, contact lenses, hearing aid, dentures

The following items will be supplied by the hospital:

  • Bedding
  • Toiletries and towels if needed
  • Hairdryers

These items are not allowed:

  • Weapons (guns, knives, multi-tools)
  • Glass items (e.g. makeup bottles, mirrors and vases)
  • Jewelry
  • Boom box
  • Razors
  • Recording devices (e.g. cell phones, tablets)
  • Products containing caffeine or alcohol
  • Drugs (prescription and illicit)
  • Smoking material (cigarettes, lighters, matches)
  • Items depicting violent and/or sexual themes
  • Toiletries (hospital provides)

Items that are sharp in nature (craft supplies, pens, pencils, tweezers, wire-bound notebooks, etc.) will be stored in a secure area and may be signed out for temporary use at the discretion of staff. We encourage patients not to bring cash in excess of $20.

All visitors must sign in at the front desk for the safety of our patients, care team and other visitors. The receptionist will ask for your name and the patient you wish to visit.  Only approved visitors will be allowed onto the units. Once approved, you will be given a numbered visitor badge. Please clip this badge above your waist where it is easily seen. 

Before leaving the lobby you will be asked to place all personal belongings (e.g. purse, coat, cell phone, keys, smoking materials, weapons, pocket knives and multi-tools, etc.) in the secure lockers in the lobby. Please take your locker key with you. You will be greeted upon arrival to the unit. Please return your visitor badge to the receptionist desk before you leave. 

MaineHealth Behavioral Health at Spring Harbor respects the rights of all patients, above all, the right to be treated with respect and dignity.
