Primary Care

All five Franklin Health primary care medical offices have received recognition for being patient-centered medical home models, a widely recognized symbol of quality that is only earned by successfully passing rigorous and thorough reviews.

Recognition demonstrates that the practices have the tools, systems and resources to provide their patients with the right care at the right time, including:

  • Same day appointments as needed
  • Expanded hours
  • 24/7 access to a provider, and
  • Direct delivery of behavioral health services by licensed clinical social workers as needed.

Research shows that medical homes can lead to higher quality and lower costs, and improve patients and providers’ reported experiences of care.

Patient Portal

Access to a patient portal is given to all patients with computer access. A patient portal is a secure online website that gives patients convenient 24-hour access to personal health information from anywhere with an Internet connection. Using a secure user name and password, patients can view their test results, office visit summaries, medication list, and request prescription refills and appointments. Patient portals enhance patient-provider communication, empower patients, support care between visits and, most importantly, improve patient outcomes.

Don’t have access? Please contact your physician practice for more information.

Our "Where to Go for Care" infographic is designed to help patients identify appropriate sites of care based on cost, convenience and medical condition. The infographic content has been vetted by clinicians, administrators, patients, and communications professionals across the MaineHealth system.
