Community Partnerships
We all need supportive relationships and this is especially true as we age. Local geriatric care facilities, community programs and support groups provide important resources that help older adults age with dignity and maintain community connections.
MaineHealth Geriatric Care partners with Southern Maine Agency on Aging, the Alzheimer’s Association and Partnership for Healthy Aging to offer educational programs and support groups designed to help older adults and their families cope with issues related to aging.
Our geriatric specialists also provide care onsite in local nursing homes and assisted living facilities. This care coordination and clinical oversight helps older adults transition from one health care setting to another - such as an acute hospitalization to a skilled nursing facility. In addition to nursing home care and assisted living care, our geriatricians also see patients at New England Rehabilitation Hospital of Portland.
