Medical Records

Franklin Memorial Hospital is enthusiastically embracing the benefits of electronic health records (EHRs) and other forms of health information technology (IT). EHRs improve patient safety and care by preventing the ordering of unnecessary tests and imaging procedures, and reducing medication errors. Electronic reminders and prompts also alert health care providers to preventive care, such as when a patient should have a colonoscopy, get a flu shot, or get certain lab tests.

Franklin Memorial Hospital has made IT in patient care a priority in recent years, implementing: HealthInfoNet, a statewide system for doctors and other health care providers to share information and improve health care; bedside medication verification; PACS, which is a system for storing and sharing digital medical images through computer workstations; and regional e-prescribing, which improves access to medications, increases patient safety, decreases medication errors, and increases the number of patients who fill their prescriptions and take their medications according to their doctor’s instructions.

How to obtain a copy of your FMH medical records

All patient information is confidential and available only to individuals directly involved in that patient’s care.

To Request Records in Person

  1. Print and complete the Authorization to Receive Protected Health Information Form.
  2. Return the form to the Health Information Management Department (HIM) (medical records), Release of Information office, located on the Ground Floor of the Medical Arts Center behind the hospital, Monday through Friday, 8:00 a.m. to 4:30 p.m.
  3. You must present a valid photo ID when picking up records.

* If anyone other than the patient picks up the records, that person must have a copy of the Power of Attorney, other legal documentation provided by the courts, or be the person named on the authorization form in the “recipient” space.

*Please call 3 business days before you need the records so that we will have them ready for you. Otherwise, the records will be mailed or ready for pick-up within 5 to 10 business days.

To Request Records by Fax/Mail

  1. Print and complete the Authorization to Receive Protected Health Information Form.
  2. Fax the completed authorization form to the Health Information Management Department (HIM) at (207) 779-2629, or mail the completed authorization form to:

* Be sure to include the address where the records are to be mailed or the secured fax number.

* After verification to confirm that the signature you have provided on the form matches the signature on file at the hospital, your records will be mailed to you within 5 to 10 business days.

Need a Copy of your Medical Records?

Go to the Health Information Management office, located on the ground floor of the Medical Arts Center, located on the hospital campus, Monday through Friday, 8 a.m. to 4:30 p.m. or call (207) 779-2330.

Records Not Available from the Hospital

  • Birth Certificates (Can be obtained from the town office in which you were born.)
  • Death Certificates (Can be obtained from your local town office.)
  • Blood Types (Blood types are not automatically kept on file at the hospital, only in those cases when the knowledge of your blood type was necessary for the visit.)
  • Immunization Records (These are usually obtainable from your primary care provider.)

Patient Portal

Access to a patient portal is given to all patients with computer access. A patient portal is a secure online website that gives patients convenient 24-hour access to personal health information from anywhere with an Internet connection. Patient portals enhance patient-provider communication, empower patients, support care between visits and, most importantly, improve patient outcomes.

Franklin Memorial Hospital is transitioning to a new online patient portal called MyChart on March 2, 2019. Our original patient portal is now read only. This means:

  • You will still see your health information in the original portal prior to March 2.

  • No new information will be added to the original portal after March 2.

  • You will no longer be able send to messages to your provider through the original portal after Feb. 27.

  • We no longer provide technical support for the old portal.

  • The old portal will be disabled entirely on May 1. 

After March 2, you can enroll in MyChart, a free secure website that lets you see your health information online. It will allow you to:

  • Request and manage your appointments

  • Review your health summary

  • View your lab or test results

  • Communicate securely with your care team

  • Request prescription refills

  • Make an online payment

At your next scheduled appointment after March 2, you will receive an activation code and instructions for enrolling in MyChart/
