MaineHealth provides leave and disability support and resources when care team members need it most
Broadspire is the third-party administrator for MaineHealth leave and disability benefits. Broadspire's secure website provides information and tools that support you as you file your claim.
How do I file a leave request or short-term disability claim?
- Contact your manager or supervisor to discuss when your leave will start and how long you expect to be away from work. You must follow your department’s usual call-in rules; simply “calling in sick” does not initiate your protected leave, nor any claim for short term disability/wage replacement.
- Notify Broadspire via phone at 877-907-5452 or on the web at https://leavetech.my.site.com/connect. If you are unable to give notice, another individual such as your spouse, parent, adult child or friend may notify Broadspire on your behalf. Broadspire will require the following information:
- Your name and social security or employee ID number
- Complete mailing address and phone number
- Date of birth
- Marital status
- Occupation (or job title)
- Manager’s name and phone number
- A brief description of your medical condition, including the cause of injury or illness, and the date the injury or illness began
- Your last day worked and your first day absent from work
- The date you expect to return to work (if you know), or the date you have already returned to work
- Whether the injury or illness is work-related
- Your physician’s name, address, fax and phone number
- The dates of your first, most recent and next scheduled visits with your physician
- Work restrictions or limitations advised by your physician, if any
- When you initiate your leave claim with Broadspire, you will decide whether to use PTO, PTA or EPL to supplement your leave benefits. This decision is final and cannot be changed later. You cannot choose to use only a portion of your PTO, PTA or EPL; supplementation must be elected in full or not at all.
- Visit the “Life Events" section of the MaineHealth Benefits Portal to make benefit changes such as adjusting your HSA, reducing or deferring coverage, or adding a newborn while you are on leave.
Once your leave or claim has been reported, contact Broadspire via phone at 877-907-5452 or on the web at https://leavetech.my.site.com/connect to access claim information, view updates or make changes to your open claims.
