We strongly encourage Master Trainer sites to register with the Matter of Balance Online Data Entry System to be a part of the national data collection. With support from the U.S. Administration on Aging and the National Council on Aging, Senior Services has developed a web-based system* that will permit all of us to continue to obtain high quality and timely data. Reports generated from this data are valuable for everyone -- participants, community sites, funders and policymakers. These reports are helping the evidence-based movement not only locally in your own communities, but nationally.
* Developed on the widely-used Salesforce platform.

We have made the forms available for Master Trainers to download and print for use with the online data entry system. If you are not registered with the system, you may still use the forms to collect your own data.

Forms for Class Participants with Low Vision
The Matter of Balance First and Last Class Surveys and Class Evaluation Forms are available in 12-point font and 20-point font for visually impaired participants.
Registration Questionnaire for Potential Participants
The Area Agency on Aging in the Heart of Texas developed a questionnaire that Master Trainers and/or Coaches can use when talking with potential class participants. This helps them to make an initial connection and understand what the participants hope to gain from attending the session. Download the registration questionnaire.

Ohio Northern University shares a Bingo game that you can use to have fun promoting falls prevention education and awareness.
Falls Prevention Information Card
Marin General Hospital of Greenbrae, CA, created this informational card to place in waiting rooms and distribute at health fairs promoting the fall prevention section of their website, including Matter of Balance classes.
Fall Prevention Interest Card
The Ventura County Area Agency on Aging Fall Prevention Program makes cards available for people to fill out at health fairs and other events. The cards provide a quick, easy way for people to check off the program(s) in which they are interested and provide contact information so that Ventura County AAA staff can follow up with them. Participant Letter to Healthcare Provider
WellSpan York Hospital of York, PA, provides a letter to A Matter of Balance participants after they complete the class for them to share with their healthcare providers. Customize a letter for your participants.
Partnership with Fire Department
San Antonio Oasis has partnered with the Leon County Fire Department to offer A Matter of Balance and other fall-prevention classes to the community.
A Matter of Balance Virtual Translation (MOB-V) Update Training for Master Trainers Now Available
We are happy to offer A Matter of Balance Virtual Translation (MOB-V) training sessions, which will enable you to train coaches virtually and offer A Matter of Balance classes online. The MOB-V model includes significant changes to allow for increased safety and adaptation to the online environment. MOB-V Translation training is required in order to train Coaches and offer MOB classes virtually.
The training is 4.5 hours long. The cost is $450.00 per person. The fee includes all training materials (new MOB-V Master Trainer Manual, Coach Handbook, Participant Workbook, digital links to the videos, and training materials to train your coaches. Shipping is included.) This training is available to current Master Trainers only. You will need a credit card to pay on the website to secure your enrollment spot. The training is virtual, on Zoom.
