- Advanced Health Care Directive
- Do Not Resuscitate (DNR)
- POLST forms
When you need medical help but are not able to speak for yourself, a written plan can make your wishes known. Our experts will explain these documents, why they are important, and how to complete them.
Presenters include Shirley Frederick, MD, Southern Maine Health Care Family Medicine and Britton Ryan Garon, Esq., LL.M., Attorney at Bergen & Parkinson, LLC.
