Our referral specialists can answer questions and help clarify insurance coverage for home health care. Referral services are available seven days per week.
Fax Referrals
Physicians and others in the health care community may refer patients via fax. Faxes may be sent to 1-207-775-5521.
Physician Order Line
For physician orders, or home health plan of care changes for your active patients, please call our Physician Order Line at 1-800-257-9099 and speak directly with a nurse. The Physician Order Line is available seven days per week.
In-service Education Opportunities
Upon request, we can arrange in-service education for professional staff on topics such as:
- Home health benefits and services
- Eligibility and reimbursement programs
- Wound care, telehealth, diabetes, rehab programs, post-operative joint replacement services, and palliative care
In addition, our Provider Relations team works with the health care community to ensure a smooth transition home from a hospital or skilled care facility. We also have assigned liaisons to community hospitals to help facilitate referral and care transitions. Face-to-Face Encounters
On January 1, 2015, the Centers for Medicare & Medicaid Services (CMS) made additional changes to the Face-to-Face (F2F) documentation requirements that are part of the eligibility standards for all Medicare home health agencies. When referring to home health, the certifying physician must include documentation that substantiates:
- Need for skilled services
- Home-bound status
- Services under a physician plan of care
- Face-to-Face encounter occurred 90 days prior to or 30 days within the home health start of care

Quality Initiatives
MaineHealth Care at Home maintains a comprehensive program for outcome-based quality improvement. Our efforts include utilization of a comprehensive bench-marking service though Strategic Healthcare Programs. In addition to these efforts, we contract with a national consulting firm to survey our patient satisfaction.
