The providers of  Memorial Hospital Mount Washington Valley Rural Health Primary Care have been serving the community since 1978. We are a multidisciplinary group of Family Practitioners with sub-specialists in Behavioral Health and Pediatrics. We offer an array of health services for prevention and treatment of acute and chronic illness for all age groups in both outpatient and inpatient settings.

Our Family Practitioners, along with our Nurse Practitioners, offer complete care from newborns to geriatrics, following standards of preventive care as outlined by the American Academy of Family Practitioners and the U.S. Preventive Services Task Force. Services also include intensive care and coronary care, exercise stress testing, echocardiography, acupuncture, and musculoskeletal manipulation.

NCQA Recognized Patient Centered Medical Home

We received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships.  They received diabetes recognition, awarded by NCQA for providing excellent care to their patients with diabetes.
 
The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.

Family Medicine

Family practice at Memorial Hospital Mount Washington Valley Rural Health Primary Care provides both in-patient and out-patient pediatric care.

Behavioral Health

We offer embedded behavioral health services from a licensed clinical social worker ensuring the best coordination of care possible between your primary care doctor and your behavioral health specialist.

All ages are welcome- newborns, adolescents, adults and elders. Routine health maintenance checks, acupuncture and osteopathic manipulation are also available. Call for an appointment. New patients welcome.
