Our program provides medical, surgical, behavioral, nutritional, fitness and other strategies for adults and children with obesity. Treatment begins with an appointment to see one of our obesity medicine specialists. Our specialist will collaborate with you, our care team, and our mutual patient to design a custom treatment plan for weight loss, wellness and disease prevention.  

Referral Guidelines

• An adult patient must have a BMI of 35 or greater to be referred to MMC Weight & Wellness
• To be eligible for surgery the patient must either have:
  • A BMI of greater than 40 or
  • A BMI greater than 35 with comorbidities (insulin resistance and type 2 diabetes, hypertension, dyslipidemia, cardiovascular disease, stroke, sleep apnea)
  • Pediatric patients must have a BMI that is greater than the 85th percentile to be referred to our program

Referral Process

If you are a MaineHealth provider, please enter referrals through EPIC AMB REF1104. If you do not have access to EPIC, please mail or fax the following information to our office:

• Patient contact information/demographics
• Emergency contact
• Insurance(s)/guarantor (pediatrics)
• Current BMI
• Medical history & physical exam notes (within the last year)
• Recent lab work (within the last year)
• Most recent EKG
