Local resources for people with memory concerns, and their caregivers.

 MaineHealth Alzheimer’s Disease Partnership offers resources to people with memory concerns and their caregivers in Greater Portland and Lincoln County.  In September 2017, MaineHealth received a large three-year grant from the Administration for Community Living.  The grant funds the MaineHealth Alzheimer’s Disease Partnership that serves Lincoln County and Greater Portland. The project’s goal is to improve MaineHealth’s dementia services through the following:

  • Providing dementia specific training to clinical staff and community partner staff
  • Increasing referrals between the health system and community-based organizations
  • Expanding existing supportive services
  • Offering new evidence-based services through partner organizations

Partners include:  

  • Southern Maine Agency on Aging
  • MMC Geriatric Center
  • Maine Behavioral Healthcare
  • Spectrum Generations
  • Lincoln Health
  • Lincoln county Para medicine teams
  • Alzheimer’s Association Maine Chapter.  

Services Available:Community support

Greater Portland include:  Adult day service scholarships, Adult day service Music and Memory© programming, and Community Support Services.  

Lincoln county include:  Mainely Delivered Meals, Personal Support Services, Music and Memory© in the home, and Caregiver Community Education sessions, Community Resource Specialist service. 

Both regions include:  The evidence-based, Resources for Enhancing Alzheimer’s Caregiver Health (REACH Community™) program is available to provide support to caregivers via Maine Behavioral Healthcare’s (MBH) integrated clinicians and MMC Geriatric Center’s social work team.  

Currently, the MBH REACH Community™ certified clinicians are located in five primary care practices and the MMC Geriatrics Center including:

  • Maine Medical Partners- Falmouth Family Medicine, Portland Family Medicine
  • SMHC- Biddeford Internal Medicine, Sanford Family Medicine, and Saco Family Medicine
  • LMP- Waldoboro 
  • MMC- Geriatrics Center Portland

All services available through the partnership are free of charge.

Alzheimers Disease Partnership

To make a referral for an individual with cognitive impairment in Greater Portland go to mainecommunitylinks.org or in Lincoln county contact Spectrum Generations at 1.800.639.1553.  To make a referral for a caregiver, contact the REACH Community™ practices to schedule an appointment with a clinician.
