Maine CHNA Community Forums provide an opportunity to discuss local health needs and data that inform regional plans to address our most pressing health needs. This is your opportunity to provide valuable input based on the 2021 County Data Health Profile. The County Data Health Profile will be sent to all registrants approximately one week prior to this meeting for you to review. We invite you to join us in this conversation so that together, we can become the healthiest state in the nation.

Community Health Needs Assessment

Join us at a community health needs assessment forum near you!

Community members from across Maine and Carroll County, NH are invited to participate in upcoming regional community health forums. These forums are part of the Maine Shared Community Health Needs Assessment (CHNA), a regional public-private partnership that occurs every three years and includes Northern Light Health, Central Maine Healthcare, MaineGeneral Health, MaineHealth, and Maine Center for Disease Control and Prevention.

The forums bring together many community stakeholders to identify priority needs, local resources and opportunities for partnership and collective action. Local hospitals, community-based organizations and public health entities use the data gathered at these forums to inform local and regional health improvement plans. MaineHealth hospitals will develop and publish health improvement plans for its health service areas in the spring of 2022.

All forums will be held online and registration is required.

Community Health Needs Assessment (CHNA) Reports

Per federal IRS regulations, not-for-profit hospitals are required to conduct community health needs assessments in three-year cycles. MaineHealth hospitals' first cycle was for fiscal years 2013-2015 (more specifically, for October 2012-September 2015).

Below, Community Health Needs Assessment reports are provided for MaineHealth network hospitals. Each report:

  • Provides county-level data on close to 200 health measures – such as rates for smoking, substance abuse, obesity, high blood pressure and childhood immunizations.
  • Summarizes conversations by diverse community stakeholders, who reviewed the data for their county and shared ideas to address pressing health needs.
  • Offers a detailed, three-year action plan developed by each MaineHealth hospital for improving public health in the communities we serve.

Annual progress reports about health improvement actions implemented are also provided below.
