If you or a covered dependent are enrolled in the MaineHealth Employee Healthy HMO Plan and are dealing with a chronic condition, you are eligible to participate in the ConditionCare program through Anthem.

As a benefit of this program, you are eligible to receive a cost savings on copays and medications related to the following conditions:
