What is a patient-centered medical home?

At MaineHealth, our primary care providers offer each individual a patient-centered medical home Patient-centered medical homes make sure patients get the care they need when and where they need it. That’s because your primary care provider will help coordinate and stay involved in your care.

Your primary care provider will make sure you connect to the services you need, such as a:

  • Specialist or specialty care
  • Mental health counseling
  • Hospital care
  • Rehabilitation therapy
  • Nursing home care.

Why choose a patient-centered medical home?

  • Individualized care: Each patient gets more time with providers, and providers get to better know the patients and their needs.
