SMH Welcomes Deb Jordan as Case Management Director

Western Maine Health Senior Vice President of Clinical Services, Margaret Burns, RN, PhD, is pleased to announce that Deborah Jordan, RN, CCM, has recently accepted the Case Management Director position at Stephens Memorial Hospital. In her new role as the Case Management Director, she will ensure consistent delivery of quality, timely, and safe patient care.

 

Deb has been a nurse since 2002 and is currently enrolled in the Master of Science in Nursing program at the University of Southern Maine. Of her new position Jordan shares, “I am excited and honored to join the team as director of case management. I hope to ensure improved patient outcomes, safe care transitions and exceptional coordination of care across the healthcare system.

 

In her free time Deb enjoys spending time with her husband, children and grandchildren.  Her hobbies include:  hiking, camping and yoga.
