Develop standardized care teams with clearly defined roles and responsibilities for each team member

Enhance role and up train MAs, RNs and LSWs

Develop a strategy to incorporate Advanced Practice Professionals

Optimize EPIC to deliver team based care\
Decrease variation in care

Adopt and implement standards of care for the 10 most common illnesses treated in the primary care setting
Improve patient experience and engagement

Understand patient preference, collect timely data and be accountable for implementation of improvement plans

Leverage technology (patient portal, virtual visits, telehealth)
Manage the total cost of care of our most complex patients

Increase focus and targeted approaches for populations that drive cost

Enhance strategies such as timely primary and specialty care consult and referral process as well as understanding costs related to laboratory and testing
