MaineHealth wants all of its patients and employees to have a voice in their care. Advance care planning is a way a patient/person can make their wishes known for care in a health crisis even if the person is not able to communicate at that time. The following information will help you get started.

What is advance care planning?

Due to an accident or illness, we may not be able to make our own health choices. Who would make healthcare choices for you then? Does the person know your wishes for care?

Planning ahead for the unexpected is normal and an important part of good healthcare. Advance care planning is a way to tell your loved ones and doctor what kind of care you want if you become seriously ill.

Talking about your choices while you are healthy makes it easier for family / loved ones during a health crisis.

You do not need to hire anyone, but you may wish to receive help with your advance care planning. The process is free, but it takes some time.
