Introduction
All outpatient Geriatric
programs are provided at the Geriatric Center,
located at 66 Bramhall Street in
Portland, only a block away from Maine Medical Center.
With its own designated parking lot, wheelchair ramp and
convenience to MMC, the Center is easily accessible for
patients, families and staff alike. In keeping with the
concept of patient and family centered care, input was
obtained from a family member regarding the design and
feel of the space. Furniture, fabric and colors were all
selected with the elder population in mind and help to
provide a comfortable and pleasant environment for
seniors and their families.
The Geriatric Center has a long
history of providing care to Maine’s elders using a team
approach and the close proximity of the outpatient space
to MMC ensures ongoing, collaborative and integrated
care for seniors. The Geriatric Team focuses on elder
health and age-related issues that can impact daily life
and offers information and tools to optimize health and
day to day functioning. The clinics presently offered at
the Geriatric Center include: Geriatric Assessment
Clinic; Memory Disorders Program; Eating and Swallowing
Clinic and a Falls Clinic. In addition to these, a
Muscular Dystrophy Clinic is held at the Center on a
monthly basis; a Pediatric Feeding Assessment is offered
weekly; Enhanced Wellness programs sponsored by the
Partnership for Healthy Aging are offered on a regular
basis; and other community programs and support groups
are also
available.
