The Target Diabetes Program is based on the Chronic Care Model and focuses on interventions aimed at improving diabetes by addressing the major components of chronic illness, which include: - Patient Education and Self Management - Healthcare Delivery Design - Provider Clinical Decision Support - Clinical Information Systems
