Living Well

The Chronic Disease elf-Management Program, or Living Well, is a six-session course designed to help older adults with chronic conditions by covering techniques to deal with problems such as frustration, fatigue, pain and isolation, appropriate exercises, medication use, nutrition and communicating effectively with family, friends and health professionals.  

This program was developed by the Stanford University Patient Education Center. In evaluating this program, randomized studies of program participants have shown improved health status, improved healthful behaviors, and decreased days in the hospital.

Other components: community programs such as fitness programs, daily hot lunch, nutrition and health education, interest groups and classes, information and referral, assistance with transportation, immunization clinics, and health screenings.
