All of these materials can be downloaded here or are available in a booklet for patients by phoning the MaineHealth Learning Resource Center at (207) 396-8570. All of these documents can be viewed with Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader, you may get a free copy by going to the Adobe Web site (you will leave the MaineHealth site) and click on the "Get Acrobat Reader" button. If you have a Palm OS, Pocket PC or Symbian OS and need to download Acrobat reader software for these mobile devices click here for the correct link within the Acrobat Web site.
