What is Care Management?

Care Management is a program to help you become as healthy as possible and to connect you to resources and information to help you manage your health. As a part of the healthcare team at your doctor's office, the Nurse Care Manager can assist you with many things, including the following: 

  • Healthy Eating
  • Physical Activity
  • Medications
  • Goal Setting and Action Planning
  • Equipment Care and Use
  • Preventive Health
  • Risk Reduction
  • Stress Management
  • Care Coordination
  • Community Resources

The Care Manager will provide you information for your health condition and any other health needs.  One thing you will work on together is setting action plans, or small steps you can take to reach your goals. The Care Manager can meet with you at the doctor's office, at home or in the community. They also work with you over the phone.

How does the Care Manager work with my primary care provider?

The Nurse Care Manager is a member of the primary care team and has a close working relationship with your primary care provider. They will regularly consult with your provider to discuss your care. If your provider feels you could benefit from this service, the Care Manager will call you to set up a time to discuss your health. You may also ask your provider to make a referral to the Care Manager. 

How long does the Care Management Program last?

It depends on your health and progress towards your goals. You are not required to accept the support and may stop at any time. 

What are the benefits of the Care Management Program?

Patients have been very satisfied with the program. Many say they have increased confidence in managing their health conditions and improved quality of life. This program has also shown improved health outcomes and reduced hospital readmissions.

What about confidentiality?

Your personal health information will be kept confidential. The Care Manager follows all patient privacy laws. 

What are the costs?

You will not be charged for any Care Manager costs.  In fact, your health plan encourages participation in this service to improve your health and wellness. 

How do I request this service?

You can ask your primary care provider for a referral.  You may also call the MHACO Care Management Department at 1-877-270-1339 (toll-free) to request the service.
