Care Management for MaineHealth health plan members

What is Care Management?

Care Management is a program to help you become as healthy as possible and to connect you to resources and information to help you manage your health status. As a part of the health care team at your doctor's office, the Nurse Care Manager can assist you with many things, including but not limited to, the following: 

  • Nutrition
  • Physical Activity
  • Medications
  • Goal Setting/Action Planning
  • Equipment Care/Use
  • Preventive Health
  • Risk Reduction
  • Stress Management
  • Care Coordination
  • Community Resource Outreach

The Care Manager will provide you education and information specific to your health condition and any other identified needs.  One thing you will work on together is setting action plans, or small steps you can take to reach your goals.  The Care Manager can meet with you at the doctor's office, at home or in a community setting.  They also work with you over the phone.

How does the Care Manager work with my primary care provider?

The RN Care Manager is a member of the primary care team and has a close working relationship with your primary care provider.  Patient-focused collaboration, consultation and clinical updates are ongoing.  If your provider feels you could benefit from this service, the Care Manager will call you to set up a time to discuss your health.  You may also take the initiative to ask your provider to make a referral to the Care Manager. 

How long does the Care Management Program last?

The length of time the Care Manager works with you depends on your health status and progress towards goals.  You are under no obligation to accept the support and may stop the service at any time. 

What are the benefits of the Care Management Program?

Consumers report high satisfaction with the program, increased confidence in managing their health conditions and improved quality of life.  This program has also demonstrated improved health outcomes and reduced hospital readmissions.

What about confidentiality?

Your personal health information will be kept confidential.  The care manager follows all patient privacy laws. 

What are the costs?

You will not be charged for any Care Manager costs.  In fact, your health plan encourages participation and engagement in this service to maximize health and wellness. 

How do I request this service?

You can ask your primary care provider for a referral.  You may also call the MMC PHO Care Management Department at (207) 482-7064 to request the service.
